New Student Requirements

Prior to registration, ALL new students accepted for admission must submit a completed UCF Immunization Health History mandatory form.

Requirements and Vaccine information

 

UCF has partnered with Med+Proctor for immunization verification.

All incoming students (freshmen and transfer) will receive an Immunization Hold, preventing course registration until all immunization requirements are fulfilled.

1. Submit the mandatory UCF Immunization Health History form signed/stamped by your healthcare provider -OR-
2. Submit the mandatory UCF Immunization Health History form along with your official vaccine records.

The Immunization Hold is automatically removed within 1 – 2 hours AFTER Med+Proctor has verified the immunization record as compliant.

NOTE: Med+Proctor is a FREE service. All fees are OPTIONAL.

Exemptions

• Online Student Exemption
  Students who are accepted in an Online-Only program are still required to submit the mandatory UCF Immunization Health History form, however, only the demographic information and signed waivers (under section A) require completion.
• Military Exemption
  Students who are active-duty or veterans may submit the mandatory UCF Immunization Health History form with only the waivers (under section A) signed and a copy of the military ID or DD214 form.
• Medical Exemption
  Students with certain medical conditions and/or contraindications to MMR vaccine, please, contact our office by phone at 407-823-2701 (opt. 4) or via email at immunizations@ucf.edu for the accepted documentation.

Student Health Services offers all the required vaccines and blood titer tests for compatible prices.
