How can I get a copy of my medical records?


The HIM department staff is responsible for the quality control of patients’ health information and the handling of all requests to release or obtain health information. The HIM department is located on the 2nd floor of the UCF Health Center (building 127). They may be reached at 407.823.2091 or by email at medrecords@ucf.edu.

A patient may request a copy of their records for themselves or to be sent to a third party (such as a referring physician, parent/guardian, or another academic entity). Nominal charges may apply for this service. In order to process the release of the health information, the requester must complete the Authorization to Release Protected Health Information form. The form must be notarized if not completed in person at the HIM department or sent from the requester’s official UCF mail account on file. Every effort will be made to release the record within 5 working days. Please include a working telephone number on all request forms in case there is a question or concern. If the patient is younger than 18 years, a parent or guardian will be required to sign the form.
